Last Diet Ever Financial Policy

PAYMENT METHOD
We accept cash, personal checks, VISA, MasterCard, Discover, and CareCredit (not available at all LDE offices; see information at right for additional details). There is a service charge of $25 for returned checks.

INSURANCE
Our physicians are not contracted providers with any insurance carrier under Last Diet Ever, LLC. The patient is responsible for payment of all services/products.

DISCOUNTS/COUPONS
Only one discount/coupon permitted per client. Restrictions apply. If using a CareCredit card for payment, use of any discount/coupon is not permitted.

CANCELLATIONS
Should you find for some unforseen circumstance you wish to cancel your program and have not yet met with our Medical Director or received your prescription, a partial refund of money can be issued (total program cost minus $150).

REFUNDS
Once you have paid for your weight loss program, met with our Medical Director, and received your prescription, there can be no refund, partial or otherwise. Once any prescription medical is dispensed, legally there can be no return. Coupons, discounts, and earned premiums (from referrals, testimonials, or promotions) have no cash value.

Any and all disputes must be filed in the Erie County Court System, Sandusky, Ohio.

Interest-free Financing AvailableCareCredit

All Last Diet Ever offices that offer CareCredit as a payment method have six and 12 month "No Interest Payment Plans" available (subject to credit approval). Applications may be made online or over the phone. CareCredit is available for use on initial program purchases and additional prescription purchases (minimum of two week supply) only. The cardholder will need to be present for all purchases and will need to bring two forms of identification: Driver's license, major credit card, passport, military ID, government issued ID, department store credit card.

Please note: CareCredit is not available at all Last Diet Ever, LLC offices.
