  • Your story (you may attach a .doc file, a .pdf file, or simply describe your personal story within the email)
  • A current photograph (please attach a .jpg file or a .pdf file)

Eligibility Requirements, Consents, & Releases

 

DESCRIPTION OF PRIZE: One four-week weight loss program utilizing prescription hCG therapy at Last Diet Ever, LLC, valued at over $500. Includes lifetime enrollment fee, consultation with our medical doctor, 28-day supply of hCG (injections), four B12 injections, and unlimited counseling/support from your own personal weight loss counselor during your four-week program. Any products or additional weeks of hCG are not included with this giveaway.

Eligibility

 

  • You must be at least 18 years of age to enter.
  • One entry per person per month accepted.
  • Entry deadline is the 20th of each month.
  • Entries will be considered only if they are complete, including a letter summarizing your personal story and a current photograph.

 

Consents & Releases

All materials you send, including photographs, will be retained by Last Diet Ever, LLC and will not be returned to you whether you are selected or not. Any expense you incur during the entry process, including postage, are your sole responsibility. Last Diet Ever, LLC will not reimburse you for these expenses. Last Diet Ever, LLC is not responsible for entries that are lost, late, or misdirected. Entries found to be false will be grounds for disqualification.

 

  • All decisions are final and absolute, and not subject to inquiry.
  • Employees of Last Diet Ever, LLC and their immediate family members are not eligible.
  • One winner will be chosen each month.
  • The winner will be notified in writing, at which time they have ten days to confirm acceptance. If we are unable to contact a winner via their given contact information, or if they decide not to participate, the giveaway is forfeited and an alternate winner will be chosen from all remaining entries.

Winner Responsibilities

 

  • The winner must execute all waivers and release agreements required by Last Diet Ever, LLC. By entering or accepting the giveaway, the winner agrees to be bound by these rules and regulations, and to comply with all federal, state, and local laws and regulations.
  • The recipient must grant permission to use their name and photograph for advertising and promotional purposes without additional compensation.
  • The winner agrees to release Last Diet Ever, LLC and its staff from liability of any kind that may arise from, or in connection with, his/her acceptance of the giveaway.
  • The winner agrees to complete the medical screening process set forth by our Medical Advisor, Susan Graham, MD, prior to initiating their weight loss program. This includes but is not limited to

    1. Complete and sign medical history form, consent form, and client responsibility statement/waiver

    2. Get blood work drawn (cost of the blood work is not a part of this giveaway; if you have had some blood work drawn within the last three months, this might be acceptable, depending upon what was drawn and any abnormal findings)

    3. Dependent on your medical history, you may also be required to get medical clearance from your family physician prior to starting your weight loss program. If you are not given medical clearance by Dr. Graham and/or your family physician, you will be deemed ineligible to do the program. Your prize will therefore become void. No substitutions or other compensation will be made.

Other Information

As with any weight loss program, results may vary and weight loss is not guaranteed. Void where prohibited or otherwise restricted by law. There are no prize substitutions or cash equivalents permitted.
